      General Information

      Hours:

      M-F, 8:30 am - 4 pm

      Intake Process:

      Public, Preschool, Day Care and Parochial school officials must contact the program directly, for a school to receive services. Screening is only provided to Lake County schools, in state-mandated grades. Preschool children aged 3 to 5 are also included.

      Program Fees:

      Preschool/ Daycare children ,ages 3 to 5 up to attendance in Kindergarten, have rates of $2.00 for each hearing screen and $2.00 for each vision screen. Children in Kindergarten - 12th grade have rates of $5.00 for each hearing screen and $5.00 for each vision screen.

      Residency: Resident of Illinois
      Other: The program provides services as requested to the public, parochial, and preschool/daycare programs within Lake County.
